2010–11 Registration Packet
All forms must be complete and legible for the application to be processed. Incomplete applications will not be processed until all requested information is submitted. If you have any questions, please call 445–8020, Ext. 299.
Milwaukee College Preparatory School is an independent charter school; all applicants must live in the city of Milwaukee and show proof of residency to be eligible to attend (copy of current utility bill, WI driver’s license or ID card).
All these items must be submitted and complete for the application to be processed:
- Student Registration Form
Note: To enter Milwaukee College Prep. K4 applicants must be 4 on or before September 1st.
- Parent/Student Commitment to Excellence Pledge, signed
- Transfer of Student Records Form (below)
- Updated Copy of Immunization Records
- Residency Requirement Submitted
- Certified copy of Birth Certificate (K4 & K5 applicants only)
Note: All K4 applicants must be 4 years old by September 1, 2009
